Glovebox Handling Surplus Plutonium Can Cracked During Surveillance

15. Description of Occurrence:
On 11/13/19 Maintenance was disassembling the 3013 can cutter inside the KIS glovebox. When the mechanic moved the can cutter collet, the collet came in contact with Glovebox Panel #8 and cracked the panel.
19. Immediate Actions Taken and Results:
All work was immediately stopped in the glovebox. RPD performed surveys and no contamination was detected in the DE room or on the workers. A timeout was taken for all work in the KIS Vault.
An Issue Investigation was held on 11/13/19 at 1530 hours to identify issues and to determine appropriate path-forward / actions relative to the event.
31. HQ Summary: On November 13, 2019, maintenance personnel were disassembling the 3013 can cutter inside the K Area Interim Surveillance (KIS) glovebox. When the mechanic moved the can cutter collet, the collet came in contact with glovebox panel #8 and cracked the panel. All work was immediately stopped in the glovebox. The Radiological Protection Department performed surveys with no contamination detected in the Destructive Examination Room or on the workers. A timeout was taken for all work in the KIS Vault. An investigation was held to identify issues and to determine the appropriate path forward and actions relative to the event.
